Care Consultations
CARE CONSULTATIONS include services that assist the person with
Alzheimer's disease or related dementias and/or their families in
planning for and dealing with all aspects of the disease. The
primary focus is one-on-one meetings between family members and
agency staff.
Care Consultations are conducted by our Quincy staff and are
available by telephone, walk-in or appointment basis, 9:00 a.m.
- 5:00 p.m., Monday thru Friday. The 30 or 60-minute
sessions are confidential.
During a Care Consultation, multiple factors impacting the needs
of the family and person with Alzheimer's are assessed and
identified, an action plan is developed with the family and, as
appropriate, with the person with Alzheimer's to specifically
address issues identified, and a follow-up plan devised.
The sessions provide information about Alzheimer's disease and
related dementias, community support, and resources and offer the
opportunity to discuss specific disease issues with our professional
staff.
